What is the Initiation Stream all about?
The Initiation Stream of the Provincial PoET Program is for homes that are making their first efforts at aligning habits, practices, and policies with Ontario’s Health Care Consent Act.Â
Over four weeks – which is called your home’s “active period” – you will work with the PoET Team to learn more about your current practices, identify opportunities for change, and test and implement different change ideas. This process is based on identified PoET Goals, and PoET’s Theory of Change.
Homes that make meet minimum requirements – related to the number and mix of changes implemented – will receive a 1-Year PoET Certificate upon completion of the four weeks.
Below you will find an information page identifying different PoET Goals, and the minimum criteria that a home must meet in order to receive a 1-year PoET Certificate.

What are PoET Goals?
PoET Goals demonstrate what achieving alignment with the Heath Care Consent Act can look like in these unique settings. All PoET Goals relate to decision making: some directly to the requirements of the Health Care Consent Act and related professional obligations, and others to more practical steps that facilitate the meeting of those requirements.
Homes that participate in the Initiation Stream of the Provincial PoET Program (such as your!) work toward meeting PoET Goals by identifying and/or creating Change Ideas.

To receive their 1-Year PoET Certificate at the end of the Initiation Stream, participating homes must meet minimum criteria. These criteria have been chosen to make sure that homes have made substantial and interrelated changes that will remain in place after your active period is done. The minimum criteria include looking at the number of changes your home has made, the types of changes your home has made, and who has been involved.

Your home has access to both the PoET Preliminaries education series and the PoET Practicals coaching series. Each series includes five different sessions that help to build knowledge and skills related to Ontario’s Health Care Consent Act.

- All sessions are live and interactive, and take place on zoom.
- Each week has a different zoom link. These links are included below, and will be emailed to your home’s primary contacts.
- The Preliminaries and Practicals take place on alternating weeks; the start date you have chosen will determine which series you have access to first.

Your home's change plan
The goal of the Initiation Stream is to help your home make substantial steps towards better alignment with Ontario’s Health Care Consent Act. These steps will involve making changes.
By the end of Active Week Two your home and the PoET Team will have created a change plan that will identify what your home does (or does not do) now, and what you will do differently in the future. These are your “change ideas.” The idea could relate to something in the environment (like a form or a policy), to the knowledge that residents, family members, staff, and physicians have related to decision making, or to the behaviours and processes that people have related to decision making.
A Change Plan Sample Template has been included below; the PoET Team will work with you in Active Week Two to complete this form, which will then be posted as your Change Plan by the beginning of Active Week Three.
